First Choice House Calls
First Choice House Calls is our newest line of service that is provided to help our patients with their quality of life and healthcare. Our goal is to help support our patients, families and other healthcare providers in meeting and/or exceeding the needs of those we serve. This very special program can assist people who are home-bound and have ongoing medical care needs. It also helps prevent unnecessary discomfort associated with travel to and from doctor’s offices and can help prevent Emergency Room visits.
What is House Calls?
House calls is a group of providers (Nurse Practitioners and Physicians) who treat the patient in their home (or wherever they call home). We are rediscovering what our predecessors practiced. There are many benefits to seeing a patient in their own environment. The modern twist is that they live not just in the family home but in assisted, memory or independent living facility. This methodology of care is a missing component in our continuum of care cycle. First Choice is so pleased to bring this much needed service to our patients today.
