Hospice Services
Our Medical Directors and Nurse Practitioners coordinate with the interdisciplinary team to manage your medical needs including pain and symptom management.
Our team of Registered Nurses and Licensed Practical Nurses help with managing your medical needs for pain and symptom management. They coordinate with the Physician and other hospice team members specific to the patient’s and family’s needs.
Our CNA team helps with daily cares such as bathing, companionship, support and help in managing the individual’s and family’s comfort level
Our Spiritual Care team meets the person where they are with their spiritual journey, serving as a non judgmental companion and a listening and helping guide to the individual wherever they desire to go
Our team of Social Workers help with emotional and physical needs, to make sure all affairs are in order and to help coordinate additional resources and support if needed.
Our Volunteer Coordinator works with our volunteers to provide services either through office support, ancillary support (cooking or sewing), or to be a companion for our patients and families
This Complementary service provides non pharmacological relief and support. We provide comfort touch and any other desired massage techniques to bring comfort to our patients and families.
First Choice has a dedicated team of nurses who cover call on weekday evenings and weekends.
First Choice Hospice provides support to an individual’s loved ones for a minimum of 13 months after the loved one has passed away. This may be through face-to-face meetings, phone support, bereavement letters and/or support groups.
